Our services include:
Skilled Nursing & Rehabilitation
Our nursing team includes registered nurses, licensed practical (vocational) nurses and certified nurse assistants who are available 24 hours a day and work with the individualized care plan created for each of our residents. Our care plans are created to reduce the amount of time needed in recovery or to reduce any suffering due to disease and illness.
Our rehabilitation services vary depending on your personal needs but can include physical, occupational, recreational, IV and respiratory therapies. Resident families are encouraged to participate in therapy sessions, caregiver education, and training to make the transition home as smooth as possible.
Physical, Occupational and Recreation therapy
Our physical therapists create programs that focus on the identification and rehabilitation of the causes of pain as well as work on how to reduce the risk of future injury. Occupational therapy includes adapting your environment or actions to help increase independence and ability with life activities. Recreational therapy ties into occupational therapy, working towards similar goals of reducing stress and anxiety due to limitations caused by illness.
Speech, language and respiratory therapy
We create treatment programs that are designed to help residents affected by a number of illnesses including brain injury, dementia, stroke, asthma, emphysema and cystic fibrosis and other complex medical conditions. Our treatment focus includes residents suffering from hearing impairment, swallowing difficulties, respiratory illness and lack of breathing capacity after surgery or a traumatic event.
Wound and Trach care
We’re proud to offer a wound care program available to treat a number of wound conditions including ostomies, wound dehiscence, diabetic arterial/ venous ulcers, pressure ulcers, and pre/ post-flap surgeries. A tracheostomy opens the airway and helps to improve breathing. Our team works to safely cleanse the stoma, changing the tracheostomy tube as scheduled and ensure that our patient is as comfortable as possible. Suction care includes scheduled removal of mucus build up due to cooler, drier air provided by the tracheostomy tube to allow for easier breathing.
Medication management and IV therapy
Medication management includes the distribution of any required medication regimens related to the specific needs of each resident, whether that need is pain management or antibiotic/ antiviral treatments. Infusion therapy is provided by our team when oral medications aren’t a feasible option for treatment or if there is a need for hydration/ nutritional support.
Pastoral services
Pastoral care services can provide comfort particularly in times of illness or crisis. At Presidential Post Acute we can arrange for interfaith support for you and your family as needed.
Nutrition services
Our dietary program conducts regular assessments to address the needs of patients with weight loss, wounds or weight gain. Our menus are planned to be satisfying, nutritional and offer variety. Special supplements, nutritional snacks and a variety of fluids are offered in addition to meals. We are also happy to provide guest trays for visiting family members.
Long term living
Traditional long-term care focuses on the comfort, assistance in daily living and medication management of each resident. This can include bathing, dressing, using the toilet, eating, and transferring from a bed to a chair. Our program promotes independence, increases in quality of life and decreases in stress or anxiety levels that living without support can bring. We also encourage residents to participate in our activities program, creating a community spirit and pride with other residents.
Hospice and Palliative care
At Presidential Post Acute we use a team approach to support the goals and values of each of our residents, working to provide relief from any symptoms and stress of serious illness. Both our palliative and hospice care team also provide support by addressing any physical, psychological, social and spiritual needs our residents may have.
