At Presidential Post Acute we offer a range of Palliative and Hospice care services to make you and your loved ones feel as comfortable as possible during difficult times.
Our palliative care team has created a program that works to treat residents suffering from serious and chronic illnesses. We ask our residents what’s important to them and what their major priorities are. Based on what our residents or their family tell us, we then develop a care plan and a strategy that meets the patient’s goals and values. Our care team focuses on helping each resident gain the strength needed to carry on with daily life. We push to facilitate relief from the symptoms and stress of the serious illnesses our residents suffer with.
Our palliative care uses a team approach to care along with the support of family members and friends where possible. Our team may include:
- Social workers
- Chaplains and others as needed or requested.
Our goal with hospice care is to support the highest quality of life possible rather than curing an underlying disease. This includes maximizing comfort by reducing pain and addressing any physical, mental, social and spiritual needs. We come up with a personalized plan each resident and their loved ones which focus on making your symptoms and pain better. Medical care is provided by one of our doctors with support being provided by our nursing team and aides that help with bathing and dressing.
Presidential Post Acute understand the importance of spirituality and religion in daily life for some of our residents. Pastoral care services can bring the comfort needed, particularly in times of crisis or illness. Our team can provide or arrange interfaith support for our residents and their families.
